The experience of pregnancy is often marked by greater adversity for women from immigrant backgrounds and those residing in low-income communities, based on existing evidence. Information on the comparative risk of severe maternal morbidity or mortality (SMM-M) between immigrant and non-immigrant women in low-income communities is limited.
A study to determine if there are distinctions in SMM-M risk among immigrant and non-immigrant women living exclusively within low-income areas of Ontario, Canada.
Ontario, Canada's administrative data, covering the period from April 1, 2002, to December 31, 2019, was the basis for this population-based cohort study. All 414,337 hospital-based singleton live births and stillbirths, occurring between 20 and 42 weeks' gestation, were included, exclusively among women in the lowest-income quintile residing in an urban neighborhood; universal health care insurance was provided to each woman. A statistical analysis was undertaken between December 2021 and March 2022.

SMM-M, the primary outcome, was a composite of potentially life-threatening complications or mortality within 42 days of the initial inpatient stay related to the index birth. The number of SMM indicators (0-3) served as a proxy for secondary outcome SMM severity. Using maternal age and parity as factors, the relative risks (RRs), absolute risk differences (ARDs), and odds ratios (ORs) were adjusted.
In the cohort, there were 148,085 births to immigrant mothers, exhibiting a mean age (standard deviation) at the index birth of 306 (52) years. The cohort also included 266,252 births to non-immigrant mothers with a mean age (standard deviation) of 279 (59) years at the index birth. The significant groups among immigrant women come from the South Asia (52,447, 354% increase) and East Asia and Pacific (35,280, 238% increase) regions. Social media marketing indicators most frequently included postpartum hemorrhage requiring red blood cell transfusions, intensive care unit admissions, and puerperal sepsis diagnoses. Non-immigrant women had a higher rate of SMM-M (171 per 1000 births, 4563 cases out of 266,252 births) compared to immigrant women (166 per 1000 births, 2459 cases out of 148,085 births). This translates into an adjusted relative risk of 0.92 (95% CI, 0.88-0.97), and an adjusted rate difference of -15 per 1,000 births (95% CI, -23 to -7). In comparing immigrant and non-immigrant women, the adjusted odds ratio of having one social media marker was 0.92 (95% confidence interval, 0.87 to 0.98); two markers had an adjusted odds ratio of 0.86 (95% CI, 0.76 to 0.98); and three or more markers showed an adjusted odds ratio of 1.02 (95% CI, 0.87 to 1.19).
Among universally insured women in low-income urban areas, immigrant women appear to experience a slightly reduced risk of SMM-M compared to their non-immigrant counterparts, according to this study. All women in low-income neighborhoods should benefit from targeted improvements in pregnancy care services.

In this study, we aim to contrast an interactive risk ratio simulation (intervention) with a conventional text-based risk information format (control) in order to examine any changes in participants' COVID-19 vaccination intentions and their assessments of the benefits and potential harms.
A cohort of 1255 COVID-19 vaccine-hesitant individuals residing in Germany, including 660 women (representing 52.6% of the sample), had an average age of 43.6 years (standard deviation 13.5 years). In a study involving a total of 651 participants, a text-based description was administered. Separately, 604 participants were assigned an interactive simulation. The simulation, compared to the text-based format, was linked to a higher probability of improved vaccination intentions (195% versus 153%, respectively; absolute difference, 42%; adjusted odds ratio [aOR], 145; 95% confidence interval [CI], 107-196; P=.01) and more favorable benefit-to-harm assessments (326% versus 180%; absolute difference, 146%; aOR, 214; 95% CI, 164-280; P<.001).
