Immunohistochemical (IHC) staining for DNA mismatch repair (MMR) proteins in tumor samples is frequently employed to direct germline genetic testing and variant categorization for patients exhibiting possible Lynch syndrome. The analysis explored the breadth of germline findings among a cohort of individuals exhibiting abnormal tumor immunohistochemistry.
Individuals with reported abnormal IHC findings were evaluated and sent for testing with a six-gene syndrome-specific panel comprising 703 subjects. Immunohistochemical (IHC) outcomes were used to delineate mismatch repair (MMR) pathogenic variants (PVs) and variants of uncertain significance (VUS) as expected or unexpected results.
The proportion of positive PV cases reached 232% (163 out of 703 samples; 95% confidence interval, 201% to 265%); remarkably, 80% (13 out of 163) of these PV-positive individuals exhibited a PV within an unexpected MMR gene location. Patients with variants of unknown significance (VUS) in MMR genes predicted to be mutated based on immunohistochemistry (IHC) results should be evaluated with significant caution regarding the interpretation of these IHC findings during variant classification.
IHC-guided single-gene genetic testing in patients with abnormal IHC findings might fail to identify 8% of those with Lynch syndrome.
